Avoiding surgery does not necessarily mean making a tear disappear. For many patients, the goal of nonsurgical treatment is to achieve comfortable movement, functional strength, and the ability to return to valued activities.

PRP is less likely to replace surgery when there is:

  • A large or retracted full-thickness tear
  • An acute traumatic tear accompanied by substantial weakness
  • Inability to raise the arm after an injury
  • Progressive loss of strength or function
  • Significant muscle atrophy
  • Symptoms that continue despite an appropriate nonsurgical program
  • A high-demand injury for which restoring tendon attachment is essential

A timely evaluation is particularly important after a sudden injury. Waiting too long to assess certain traumatic tears can allow the tendon and muscle to undergo changes that may affect future treatment choices.

An injection alone rarely addresses every factor contributing to shoulder pain. Rehabilitation remains one of the most important components of rotator cuff treatment without surgery.

Temporary Activity Modification

This does not always mean complete rest. It may involve temporarily reducing painful overhead lifting, throwing, pressing, or repetitive reaching while maintaining comfortable movement.

Progressive Strengthening

A rehabilitation program may gradually strengthen the rotator cuff, shoulder blade muscles, upper back, and surrounding tissues. Exercises should be progressed according to symptoms and functional ability.

Mobility Exercises
